Job Description Nemours is seeking a Care Coordinator RN for our Cardiology team in Jacksonville, FL. The Cardiac Center High Risk Nurse (RN) Coordinator is responsible for providing specialized professional nursing care to patients with complex congenital heart disease by providing direct and indirect care in collaboration with the physicians, advanced practice providers, nursing, social workers, genetic counselors, nutrition services and other clinical staff including but not limited to admission and discharge planning of all patients admitted to the comprehensive cardiac care unit. This position will work in a multidisciplinary team to innovate, improve, and advance the quality of care through a patient centered approach. Essential Functions Delivers care in a nondiscriminatory manner that is sensitive to and values diversity and serves as a liaison between patients, their families, and members of the healthcare team. Provides chronic care management services on a regular basis to assess patient compliance to care plan, health status by completing a nursing assessment; Performs telephone triage, explains, and confirms understanding of prescribed treatment regimens/plan of care and confirms compliance to same, notifies appropriate physician and/or advanced practice provider of abnormal findings and coordinates with other disciplines to ensure patients receive appropriate and timely care. Attends daily surgical rounds, weekly case conference, discharge planning conference and oversees the execution of the discharge plan. Including, but not limited to coordinating, communicating, and monitoring patient compliance as it relates to follow up care, home surveillance of complex cardiac patients, ensuring necessary medications and durable medical equipment is available upon discharge and other social needs are met. Serves as the lead in teaching parents and other caregivers how to use the Nemours Cardiac App, monitors the daily data transmitted to the Cardiac Dashboard and communicates clinical information as appropriate to the patient’s primary physician. Identifies care needs and collaborates closely with other disciplines effectively to ensure ongoing needs are met across the continuum of care by: Collaborating Cardiac Center Social Work to ensure smooth and effective transitions of care. Assists Registered Dieticians in addressing patient and family needs while in the hospital and post discharge. Identifies any nutritionary issues, such as non-compliance with recommended nutrition guidelines and communicates issues to the Registered Dietician and other members of the care team. Assists Clinical Psychologist in addressing patient and family needs while in the hospital and post discharge. Participates in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety. Including, but not limited to data collection in support of clinical cardiology and cardiothoracic research including data collection and input of data into the NPC-QIC Collaborative database. Meets with all patients prior to planned surgical admissions to establish repour and assists patient by facilitating presurgical admission activities to ensure a smooth flow and exceptional patient experience during the process. Other duties as assigned. Job Description Nemours is seeking Nursing Assistant to join our team in Wilmington, DE for our Short Stay Observation Unit. Position is part-time, casual, 48 hours per six week block night shifts with weekend and holiday rotation. 4 West is a 24-four-bed limited-stay unit that typically cares for general medical surgical patients whose care is driven by protocols. The unit consists of single patient care rooms, which can also be isolation rooms. The goal of the unit is to provide very high-quality, high-value patient care within a 72-hour time frame. Each patient room is equipped with oxygen, compressed air and suction, along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the health care team to access a patient’s electronic medical record. These computers are also used in the barcoding administration of medications which ensures patient safety by verifying that the correct medication is given to the appropriate child. Job Description Nemours is seeking a Nursing Assistant for our 3 West floor! Position is full-time - 60 hrs every 2 wks, 12-hr rotating shifts, every third holiday, two weekends out of six. This position is responsible for: Assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Responsibilities Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Registered Nurse - Day Medicine

Job Description Nemours is seeking a Day Medicine Registered Nurse. The Day Medicine Unit has 10 physical bays that can accommodate 10 patients at one time. The Sedation Suite houses two anesthesia induction rooms, two sedation induction rooms and eight recovery bays. We have just expanded to also have the PET/MR Suite which includes one anesthesia and one sedation induction room. Hours of operation for Day Medicine are between 0600 and 1930, Monday through Friday. After hours, weekend and holiday service for sedation is provided by a rotating call team of two Day Medicine registered nurses. The registered nursing staff in Day Medicine cares for approximately 650 patients each month. Day Medicine performs single or ongoing outpatient IV infusions for many medical conditions. Day Medicine also services all of the needs for pre-sedation and anesthesia preparation, intra-procedural care and subsequent sedation / anesthesia recovery for all Medical Imaging diagnostic studies and interventional procedures as well as minor procedures occurring outside of the operating room, emergency room and intensive care environments. Registered Nurse-Primary care

Job Description Nemours is seeking a RN 36 hours a week to join our primary care team in Milford, DE. This Registered Nurse (RN) position is a 36-hour per week role supporting ambulatory care operations, scheduled Monday through Friday with one evening shift per week until 6:30 PM. The role requires flexibility to support building hours of Monday–Thursday 7:30 AM–6:30 PM and Friday 7:30 AM–5:00 PM, along with a rotational commitment of at least six Saturdays per year during hours of 7:45 AM–3:30 PM. The ideal candidate is adaptable, team-oriented, and committed to providing high-quality, patient-centered care. Primary Functions The Registered Nurse is a clinically competent practitioner of pediatric, family centered nursing care. The nurse provides safe nursing care to the pediatric primary care patients with minimal guidance and bases practice on identified principles and standards of care. The RN is responsible for daily patient care, assessment and treatment in collaboration with the clinical medical provider. The RN must be competent in caring for patients with moderate to complex heath issues. Assists nurse manager or Office manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols. Essential Functions: Follow the nursing process to deliver care: assessment, planning, implementation and evaluation. Synthesizes patient assessments data and recognizes complex situations prior to communicating with the care team. Documentation reflects ongoing evaluation and effectiveness of patient care. Provides care that meets professional standards of practice and meets regulatory requirements. Safely administers medications and medications as prescribed by the provider. Triages patient’s calls utilizing Barton Schmidt pediatric protocols, national teleheath standards and clinical advice from the clinical provider. Assists the clinical provider with in office procedures within the scope of the nurse’s license. Performs phlebotomy to obtain blood specimens and back office laboratory tests. Provides age and developmentally appropriate care to primary care patients. Additional Requirements: RN Required, BSN preferred.
