Adheres to Nursing Policies and Procedures and Organizational Goals and Objectives. Essential Duties and Responsibilities Clinical Practice Provides age appropriate care for the population served. Gives consideration to growth & developmental phase of life, to age related factors such as thermal regulation, medication administration, nutrition, safety and functional needs of the patient. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs. Ensures attention to patient’s physical, spiritual and emotional well being. Demonstrates competency in assessment and re-assessment skills of the Surgical and Medical ACC patient, plans and initiates care, and evaluates the effectiveness of interventions taking appropriate actions to move patient toward reaching goals of care. (Includes admission and post procedure assessments and updates needs of Medical ACC patient with each subsequent visit). Performs pain assessments and intervenes to optimally meet patient’s needs for comfort. Plans clinical care based on patient’s needs and uses available resources. Performs clinical care in accordance with policy and procedure. Performs peripheral venipuncture and peripheral and central vascular access care according to policy & procedure. Performs phlebotomy with IV starts and/or with venipuncture according to policy and procedure. Identifies situations that warrant patient rescue and takes appropriate action, e.g. Rapid Response or Code Blue/Pink. Adheres to the 5 rights of medication administration and uses the Medication Administration Check (MAK) system per policy and procedure. Integrates principles of infection control & infection prevention into practice. Adheres to aseptic technique as appropriate. Uses resources and references for reviewing policies and procedures, medications, and clinical information. Prioritizes and organizes work effectively within assigned shift. Performs post procedure telephone calls to all discharged patients and documents in chart. Communication Uses effective communication during hand-offs (e.g. Shift report, transfer report, etc). Organizes information into a meaningful format, such as SBAR. Delegates appropriately to unlicensed personnel and team members. Communicates patient care information to the interdisciplinary care team in a timely manner. Provides patient and family education in a manner that is understood and gives the patient and family handouts, brochures and resources that can be taken home for future reference. Accepts assignments and performs as a team member to provide best outcomes for patients. Introduces self to patient and explains procedures to patient and family. Documentation Demonstrates writing skills clearly that are appropriate for healthcare communication. Utilizes only approved medical abbreviations. Documents patient care in an accurate, professional and timely manner using the electronic medical record, MAK, and paper documentation forms according to policy and procedure. Documents patient care that is relevant to patient’s diagnosis, problems and reflects patient’s status. Obtains results from Soarian, CDC labs, Quest, Lab Corp, Interdoc, etc. Professional Role Development Develops own nursing knowledge via practice, observation, professional literature, networking and attending continuing education programs. For example, quality control measures, emergency equipment checks, medication preparation area cleanliness, monthly outdates, refrigerator logs, etc. Adheres to unit programs to support patient care such as maintaining a quiet environment, hourly patient rounds, customer service, etc. Attends staff meetings on a regular basis. Keeps patient care area and work environment neat and organized. Education Training and Experience Bachelors of Science in Nursing (BSN) is required. Experience preferred, however training and specialty specific education programs are provided during orientation, and must be successfully completed as scheduled. Basic computer skills: keyboard, mouse use. Ability to complete data entry on clinical documentation screens. Ability to use email and internet. Ability to successfully complete on the job training for CCH computer programs. Ability to practice critical thinking and organizational skills . Ability to speak, read, write and understand English. Communicates with all customers including providers, office staff, patients and fellow employees in a professional manner while keeping customer service a top priority. In collaboration with management, oversees all care coordination activities performed by the clinical staff in the team based care model. Assist in implementing, monitoring and actively participating in performance improvement initiatives to improve overall patient experience and quality of care. Effectively manages patient complaints in collaboration with Manager. In collaboration with Manager, completes annual performance evaluations for assigned staff. Coordinates coaching and or disciplinary action in accordance with LG Health and practice policies. Conducts routine clinical staff meetings. Communicates with staff on a regular basis to keep them informed of any new information, processes or procedures. Meets one on one with Lead Office Nurses at a minimum of once a quarter. Attends and actively participates in a variety of meetings including but not limited to practice staff meetings and Nurse Supervisor meetings. Clinical: Provides clinical advice by phone electronic correspondence based on clinical judgments and or triage protocols in accordance with regulatory requirements and standards of practice. Consults with provider as necessary. Provides patient specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications and other areas pertinent to patient outcomes. Greets and shows patients to exam room and prepares patient for visit with provider. Following established rooming standards, verifies patient information by interviewing the patient to obtain pertinent medical information per department protocol. Accurately documents medical and social history and purpose of visit in the patient chart. Prepares patients for exam by measuring vital signs, such as pulse rate, temperature, blood pressure, weight and height and accurately records information in patient chart. Reviews and updates current medication list making changes as needed. Addresses medication concerns with patient and provides education on possible side effects. Administers injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements and standards of practice. Prepares and assists providers with various procedures, minor surgeries and other exams as designated by the provider. Implements standing orders as established by the medical group. Performs various tests upon the request of the provider including but not limited to hearing tests, EKGs, breathing treatments and vision tests. Collects blood or other laboratory specimens and prepares them for further testing. Authorizes prescription refills as instructed by the provider Assists with test tracking and monitoring overdue results. Removes sutures and changes dressings on wounds as necessary. Assures exam rooms are adequately supplied and cleaned after each patient visit. Maintains and cleans medical instruments and equipment after each procedure. Assists with reviewing and revising patient care plans in collaboration with provider and patient and assesses patient progress. Follows up with patient between visits as necessary. Assists with proactive outreach for patients requiring preventive and chronic disease services. Performs care coordination activities among specialists and transitions of care to obtain all necessary patient information. Assists in coordination of referrals, education and community resources. Participates in daily huddles with the provider when needed. Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures. For Regional Positions: Travels regularly to various sites within the region to support operational needs as listed above. Assists at new practices and sites by mentoring and leading other team members during the initial onboarding period. Special projects as assigned. The following duties are considered secondary to the primary duties listed above: Lead, promotes and actively participates in new practice initiatives and projects as outlined by practice leadership. Assists practice leadership in managing expenses within budgetary constraints. Orders medical and or pharmaceutical supplies within budgetary allowances and inventory control Other duties as assigned by management. Minimum Required Qualifications: Current licensure as a Registered Nurse RN or Licensed Practical Nurse LPN , issued by the PA State Board of Nursing is required. Three 3 to five 5 years of relevant nursing experience. For Urgent Care, three 3 years of nursing experience in an Emergency Department may also qualify. AHA CPR BLS certification required within 30 days of hire. For Regional Positions only - Valid driver’s license is required. Prior supervisory experience in a physician office setting.
