The RN Case Manager must be adept at driving throughput metrics, clinical effectiveness, and fiscal responsibility. Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Tasks: Reviews all cases within 24 – 48 hours or the next business day of admission/bed placement and each day throughout the stay to facilitate care progression to establish an anticipated length of stay and transition planning needs. Collaborates with the medical team to formulate a treatment plan to include care transitions and promote patient flow. Completes an initial assessment of all admissions/observation patients to identify barriers that impact the length of stay and discharge planning. The Wound Ostomy RN provides expert assessment, consultation, and direct patient care for individuals with complex wound, ostomy, and continence needs. This role serves as a clinical resource to nursing staff and interdisciplinary teams, supporting best practices and quality outcomes across the hospital. This is a per diem day position. Job Description: Key Responsibilities Perform comprehensive assessments of patients with acute and chronic wounds, ostomies, fistulas, drains, and skin‑integrity concerns. Develop and implement individualized treatment plans in collaboration with physicians and the interdisciplinary team. Provide direct patient care, including advanced wound treatments and ostomy education. Conduct patient and family teaching related to wound and ostomy management, appliances, and long‑term care strategies. Serve as a resource and consultant to nursing staff and clinical teams regarding wound/ostomy care best practices. Participate in product evaluation, quality improvement initiatives, and development of clinical protocols. Document clinical assessments, treatment plans, and patient outcomes in the electronic medical record. Support hospital-wide initiatives to reduce pressure injuries and improve skin‑integrity outcomes. Qualifications Current Massachusetts RN license required. ADN with relevant experience required; BSN strongly preferred. WOCN (Wound, Ostomy, and Continence) certification required. Minimum 2 years of clinical nursing experience; wound/ostomy nursing experience strongly preferred. Strong clinical assessment skills with the ability to work independently and collaboratively. Excellent communication, teaching, and problem‑solving abilities. Experience with computer systems required, including web based applications and some Microsoft Office applications, which may include Outlook, Word, Excel, PowerPoint or Access. As an RN in Endoscopy, you will assume responsibility to lead the Endoscopy patient care team in providing quality patient care. Assesses, plans, implements and evaluates patient care from admission to discharge. Assists the physician with diagnostic/therapeutic endoscopic procedures and monitors the physiological status of the patient throughout the pre, intra and post-procedure periods. Maintains departmental on-call responsibilities during off-shifts, weekends and holidays, on a rotating basis . Acts as a role model to staff members and directs, assigns and supervises ancillary staff team members in providing the appropriate level of care most efficiently. Is able to effectively manage the team work concept and remains flexible in response to changes. Demonstrates a commitment to evidence-based practice and performs by the philosophy, established policies and procedures, and standards of the Nursing Division, Endoscopy Department, and the Hospital. 36 hours a week, Full-time day shift, Monday-Friday. On-call is required . The average amount of on-call is 4-5 shifts per month. Weekday hours will be either 5:30 pm-11 pm or 11pm-6 am and weekend and holidays are 7am-7 pm or 7 pm-7 am. You are paid an hourly on-call rate, and if called in, you will be paid time and a half. Education/Experience: Graduate of an accredited school of nursing BSN preferred Previous Endoscopy, ED, Surgical &/or ICU/CCU experience preferred Previous IV insertion experience preferred. Previous experience with Procedural Sedation preferred Certification/License: Current Massachusetts RN Licensure Basic Life Support ( AHA) Advanced Cardiac Life Support (AHA) Pay Range: $36.00 - $79.12 This position is covered by a collective bargaining agreement.
