The goal of this program is to reduce suffering and improve quality of life for these patients and their families in collaboration with other members of the health care team. As a member of the Palliative Interdisciplinary team, this position will play a pivotal role in providing palliative medicine consultation for seriously ill patients and their families. Duties include comprehensive medical and psychosocial evaluation of patients and families, skilled and meticulous symptom management, and involvement and participation in discharge planning and continuity of care efforts according to NSH standards of care. Educational efforts in palliative medicine are key elements of this work. Promotes excellence in care and customer service to all patients served.
