The Delmar Campus has two 25 bed units that provide care to anyone requiring treatment in depression, anxiety and behavior health recovery. We have a multidisciplinary team that consist of behavior health nurses, psychiatrist, Medical physicians, social workers, activity staff, music therapist and pharmacy. Preferred Qualifications Role Purpose Under the direction of manager, plans, directs, and coordinates activities of a unit on a designated shift. Performs direct patient care as needed and functions as clinical resource to nursing and support staff. Assesses, plans, evaluates and documents the nursing process of assigned unit as needed. Responsibilities Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting. Critically assess patient data to proactively mitigate impending changes in condition and establishes consults to interprofessional team as appropriate. Leads the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver in accordance with the established guidelines and standards of nursing care. This unit allows nurses the opportunity to gain experience caring for a wide variety of patients while also specializing in free flap reconstruction, surgical airways, and extremity replantation. Nurses use their assessment skills to care for post-surgical patients and are heavily involved in patient teaching. The Surgical Services Department provides pre-op and post-op inpatient care to those patients undergoing surgical procedures in the areas of ENT and Plastic surgery. ¿ Inpatient care in this department focuses on pre-op needs, post-op care and treatment after phase I recovery through discharge from an inpatient nursing division. The department's surgical observation unit provides more intensive care and treatment to those post-op or post-traumatic patients whose condition requires treatment at this level. Preferred Qualifications Role Purpose Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care. Responsibilities Promotes patient and family centered care in a healing environment. Educates patients and their families on how to manage their illness or injury, including post treatment home care needs and medication administration. Participates in activities that promote patient safety, quality and regulatory compliance. Participates in professional development. Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plans and ensures communication of the plan of care across the continuum of care. Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care. Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
