Position works 4 10-hour shifts, with one being remote. Position does not require weekends or holidays. Located at Scripps Green Torrey Pines Clinic, with occasional site travel as needed. We invite you to join our highly engaged team of Specialty Nurse Navigators. As a Specialty Nurse Navigator, you will be responsible for: Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient. Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed. Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Providing disease and treatment specific education tailored to meet patient’s individual goals of care. Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation. Provide direct, hands-on nursing care, as needed. Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assisting patients with social services needs by identifying resources in the community and referring to these programs. Collaboration with system wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care. Required Education/Experience/Specialized Skills BSN plus 3 years’ recent clinical nursing experience in a variety of clinical settings. Required Certification/Registration CA RN License BLS if working in a clinical setting. ONS Chemotherapy and Biotherapy. Preferred Education/Experience/Specialized Skills/Certification MSN. OCN if working in Hematology, ONN-CG if working in Oncology area. Ambulatory experience preferred. Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty. The PCU / DOU opened in 2014 with 36 beds, all private rooms. As a designated stroke unit for stepdown population, our highly engaged nurses provide exceptional patient care in a compassionate, welcoming environment. Our nurses thrive in a culture that is team-oriented and focused on quality care and patient experience. The ideal candidate will have at least 6 months experience working in PCU or Stepdown Unit. We invite you to join our team of Registered Nurses responsible for providing direct patient care within the scope of his/her practice. Patient care includes assessment, planning and implementing a plan of care and evaluating patients' progress towards expected outcomes. With quality patient and family-centered care in mind, the RN takes primary responsibility for the patients' care as delivered by the Care Team under his/her supervision. Works collaboratively with other members of the patient care team to ensure optimal patient care. This is a Full Time (72 hours per pay period), 12-Hour Night Shift, position located at Scripps Memorial Encinitas.
