The 24/7 Cancer Care Clinic features an outpatient treatment area for patients to come in for scheduled appointments to receive chemotherapy, blood products, IV hydration, central line and port care, phlebotomy, etc. The scheduled infusion space is staffed with staff nurses and patient care technicians. There are also Nurse Practitioners in the Cancer Care Clinic who see patients in a patient care room setting within the clinic for symptom management concerns. This allow Oncology patients from the Siteman Cancer Center to receive care for some urgent needs. These patients are managed by the Nurse Practitioners and cared for the the staff nurses and PCTs. Additionally, when patients need to be admitted to the hospital, the Cancer Care Clinic begins the care for some of these patients who meet specific criteria. Preferred Qualifications Role Purpose Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care. Responsibilities Promotes patient and family centered care in a healing environment. Educates patients and their families on how to manage their illness or injury, including post treatment home care needs and medication administration. Participates in activities that promote patient safety, quality and regulatory compliance. Participates in professional development. Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plans and ensures communication of the plan of care across the continuum of care. The role involves evaluating, diagnosing, managing, and treating various developmental and behavioral conditions from birth through age 13 years old. The Advanced Practice Provider would see patients in the following three clinics: High Risk Infant Follow-Up Clinic (evaluating children born premature and/or with medical problems at birth for developmental assessments over the course of their first three years of life), the Young Childhood Program (assessing children birth through kindergarten for the following concerns: developmental delays, autism, ADHD, anxiety, challenges with sleeping, eating, behavior, disciple or temperament), and the Middle Childhood Program (assessing children in elementary and middle school for conditions such as ADHD, learning difficulties, anxiety, autism, tic disorders, and anxiety, mood or disruptive and aggressive behaviors), including medication management. Nurse Practitioner (NP): The Nurse Practitioner functions under the general Nurse Practitioner Standardized Process Protocol approved by the Interdisciplinary Practice Committee (IDPC). The RN First Assistant directly assists the surgeon and practices under the supervision of the surgeon during the intraoperative phase. The RN First Assistant must adhere to standardized procedures and not concurrently function as a scrub nurse. Recognizes and considers age-specific needs of patients. Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds. Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary. Facilitates the coordination of inpatient and outpatient care and services as needed. Facilitates collaboration between providers and coordination of community resources. Ensures compliance with legal, regulatory and clinical policies and procedures. Participates in quality improvement initiatives. Provides and coordinates patient teaching and counseling. Supervision: Supervision is provided by the supervising/sponsoring physician with clinical privileges at LPCH, or designated attending physicians in the Clinic, Service or Department. Standardized procedures, approved by the supervising physician(s) and the IDPC, are a mandatory mechanism of supervision. Other mechanisms of supervision used may be: Direct on-site, electronic or phone supervision by a supervising physician. A physician cannot supervise more than four (4) NP/PAs at one time. Chart screens on random charts by the supervising physician or designee. Chart audits on random charts as an integral part of selected quality improvement programs. Ongoing Professional Practice Evaluation (OPPE) through tracking of performance indicators. The NP/PA will receive a formal annual performance evaluation by the Lead Advanced Practice Provider (Lead APP), Director of Advanced Practice or supervising manager/director with input from the Lead APP, supervising physician(s) and/or physician delegates, APP peers and the supervising manager/director.
