Description Job Summary: The Licensed Practical Nurse (LPN) provides direct and indirect nursing care to patients and families in routine nursing situations within the LPN scope of practice. Under the supervision of a registered nurse or licensed provider, the LPN assists in patient care, treatment, and comfort. This role is an integral part of the healthcare team, ensuring quality care in alignment with Emory Healthcare's patient and family-centered care standards. Primary duties and responsibilities: Patient Care & Clinical Duties 1. Provide patient care based on an individualized plan delegated by the Registered Nurse/Physician. 2. Monitor and document patient conditions, including vital signs, wound care, drains, and lab values. 3. Assist with medical procedures, treatments, and medication administration as per scope of practice. 4. Collect and document patient data using specialized equipment as needed (e.g., glucometer, pulse oximeter). 5. Maintain a clean, safe, and therapeutic environment following safety and infection control protocols. Communication & Collaboration: 1. Establish and maintain communication with the healthcare team, patients, and families. 2. Inform the supervising Registered Nurse/Physician about patient activities, observations, and changes in condition. 3. Provide patient and family education related to health conditions, treatment plans, and home care. Documentation & Compliance: 1. Ensure accurate and confidential maintenance of patient medical records. 2. Adhere to universal precautions and patient safety standards in all interactions. 3. Follow Emory Healthcare's policies, procedures, and regulatory requirements. Quality Improvement & Professional Development: 1. Participate in quality improvement initiatives and adhere to best practices in patient care. 2. Conduct or assist with in-service training and continuous learning opportunities. 3. Overview The LPN is responsible for performing a wide variety of nursing care, patient activities, and accommodative services for the comfort and well-being of patients in accordance with physicians' orders and under the guidance and direction of the Registered Nurse. Responsibilities • Performs basic nursing care and patient procedures as required. • Participates in conferences and serves on committees as requested. • Observes, reports, and documents changes in patient condition, observations made, complaints voiced, and nursing care performed. • Participates in patient and family health education as necessary. • Provides a safe and secure environment for patients. • Prepares for and assists with diagnostic and therapeutic treatments. • May administer I.V.F. in accordance with the State Nurse Practice Act. • Checks, irrigates, and maintains drainage tubes as needed. • May insert N/G tubes after training (KY & VA). • May administer tube feedings. • Suctions patients according to hospital policy and procedure. • Functions in emergencies and on the code team as designated. • Performs catheterizations and provides catheter care. • Administers oral, intramuscular, intradermal, subcutaneous, I.V., P.B., and approved medications. • Administers I.V.P. medications as approved (Virginia only). • Cares for patients receiving oxygen therapy and analyzes oxygen concentration of O2 tents, isolettes, and croupettes. • Administers respiratory treatments (I.P.P.B.). • Applies sterile and unsterile compresses and bandages of all types and styles. • Answers call lights and assists patients with personal hygiene and activities of daily living. • Tests urine for sugar, acetone, and pH, weighs patients, and obtains vital signs. • Cares for patients in isolation. • Assists with physical and vaginal examinations and obtaining Pap smears. • Administers post-operative care, including coughing, deep breathing, turning patients, and obtaining vital signs. • Prepares patients for surgery, including completion of pre-operative checklists. • Applies ace bandages, all types of traction, hot water bottles, and ice caps. • Measures and records intake and output. • Collects specimens, including blood, urine, stool, and sputum. • Administers post-mortem care and transportation. • Administers enemas, including retention, cleansing, therapeutic, and colonic irrigation. • Maintains cleanliness, safety, and efficiency in nursing care. • Responds appropriately in emergency situations, providing patient care, initiating CPR, and performing duties designated by the RN or physician. • Assists with the care and security of patient valuables. • Checks narcotics and barbiturates, reports discrepancies, completes incident reports, and submits documentation to appropriate management. • Cares for patients with radium and assists physicians with removal of radium. • Prepares used equipment for re-sterilization. • Cleans and maintains storage of equipment and requests repairs as appropriate. • Follows up with physicians and staff regarding equipment performance. • Complies with hospital policies for obtaining, documenting, and following through on telephone and verbal orders. • Monitors glucose using a glucose monitoring device. • Collects Quality Improvement data as requested. • Uses universal precautions. • Performs other related duties as assigned. Qualifications Education • Graduate of an accredited school of practical nursing. Licenses & Certifications • Must maintain current licensure in the state of practice. • Must successfully complete the I.V. Therapy Course. Experience • Six (6) months to one (1) year of on-the-job training is required to attain proficiency in the performance of assigned duties. Description P osition Title: Licensed Practical Nurse-Greenwich Village Clinic Nursing-Mount Sinai Hospital-Full Time-Days Position Summary: The practice of nursing as a Licensed Practical Nurse is defined as performing tasks & responsibilities within the scope of supportive & restorative care; medication administration and specialized tasks under the direction of a Registered Professional Nurse of licensed or otherwise legally authorized physician or dentist. Responsibilities A. Clinical/Technical/Service Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment.
