Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility. Detect and correct situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Report all equipment malfunctions and breakdowns to the ADON as soon as possible and keep the ADON informed of supply needs and equipment needing replacement. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Summary: The Registered Nurse (RN) provides direct nursing care to residents and supervises nursing assistants to ensure high-quality care in compliance with federal, state, and local regulations. Key responsibilities include planning and overseeing daily nursing activities, administering medications, completing required documentation, and maintaining patient confidentiality. The RN also performs professional nursing services such as catheterization, wound care, and IV care, while fostering a warm and collaborative environment. Additionally, the role involves coordinating with physicians, ensuring safety protocols are followed, and maintaining effective communication with residents, families, and staff to meet residents' needs. Be able to perform all functions under your active license as it pertains to your assigned duties. Know the residents you serve, providing care based on their care plans and physician orders. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. Assist in all aspects of a resident’s daily life, including activities of daily living, meal service, and social engagement. Appropriately communicate with clinical leadership when a resident’s needs change. Know how to approach and care for residents with dementia. Qualifications: Registered Nurse (RN) with a current unencumbered state license. Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. Experience in Long Term Care is preferred. Willing to take additional training or education, which specific states may require. CPR and First Aid certifications are required. Ability to be flexible and adaptable while handling multiple priorities. Possess written and verbal skills for effective communication. Competent in organizational and time management skills. Demonstrates good judgment, problem-solving, and decision-making skills.
