Position Summary The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners. The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process. Position Overview The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM. The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.
