GENERAL STATEMENT OF POSITION: Provides nursing care to residents in Memory Care community’s through assessment, planning implementations and reevaluation of resident needs. Carries out and interprets policies, procedures, local, state, and federal regulations. Provides person-centered physical and psychological support, care, and service to all residents and their families. Provides leadership to CNAs, and Resident Assistants. REPORTS TO: Clinical Services Director and RN Essential Functions: The following duties are normal for this position. These are not exclusive or all-inclusive. Other duties may be required and assigned. Assesses resident needs, and provides necessary nursing care as per resident's care plan. Directly supervises CNAs on one's assigned team. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance, addressing complaints and resolving problems. Responsible to participate in Pathways of Purpose program by applying functional engagement during workday in all daily interactions with residents . Ensures that resident rights are maintained Administers prescribed medications and treatments in accordance with approved nursing techniques and per physician orders. Documents nursing history and physical assessment for assigned patients upon admission; Handles resident, family, or physician complaints/concerns, referring issues to CSD as necessary. Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed. Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol. Participates in department and/or unit performance improvement activities. Other duties as assigned § Attends in-service training programs for department specific and other relevant programs, i.e., safety, infection control, orientation, etc., as scheduled. § Is aware of and carries out general community and specific departmental safety policies; always reports hazards/safety issues promptly. § Is aware of and practices principles of infection control; reports infection control issues promptly. MINIMUM TRAINING AND EXPERIENCE: A current and valid nursing license. A minimum of three years of experience in a senior living community; a minimum of three years of experience in a management/supervisory position or any equivalent combination of training and experience which provides the required skills, knowledge and abilities CERTIFICATION AND CLEARANCE REQUIREMENTS: · First Aid and CPR certification · LifeScan criminal record clearance or criminal record exemption, as required by law · Health evaluation to ensure ability to perform duties of the position and the absence of any communicable disease, including tuberculosis · Basic Class “C” Driver’s License with a clean driving record, and free of restrictions or limitations as required by State regulation IND123

GENERAL STATEMENT OF POSITION: Under general supervision, performs and/or supervises delivery of health services for all residents; directs and manages staff to ensure the safety and well being of the residents. ESSENTIAL FUNCTIONS: The following duties are normal for this position. These are not exclusive or all-inclusive. Other duties may be required and assigned. Know and understand the vision, purpose, values and goals of Morningside Place Memory Care. Assesses health and safety needs of residents; oversees medical care (medications/treatments); Assures that social model of assisted living is in operation; Assists with family contact when medical issue is involved and provides information about residents' conditions and discusses alternative actions; Provides assessment input about residents to the clinical services director; Participates in the preparation for the Department of Social Services annual inspection; Assists in development of annual strategic plan goals for the health issues in the resident services department; actively participates in the total strategic plan goals for the resident services department; Acts as a back-up to the director in his/her absence; Attends in-service training workshops, educational classes, and meetings; Responds to emergency situations; Acts as liaison with doctors and residents; Conducts work tasks safely and in compliance with the community safety program; Provides effective and courteous service to all residents, guests and co-workers; Assists in other departments as needed; Schedules transportation for residents to appointments if resident is unable; Performs other related essential duties as required. MINIMUM TRAINING AND EXPERIENCE: A High School Diploma, vocational/technical training; at least three years of experience, preferably in a geriatric setting; or any equivalent combination of training and experience that provides the required skills, knowledge and abilities. Knowledge of: Policies, procedures, methods, and practices of the community; Medical terminology; Methods and procedures involved in handling emergency situations; Use of medical supplies and equipment; Medication dispensed to residents; Supervisory methods and techniques; Records, graphs, reports, etc. used in the daily performance of duties; Principles and practices of nursing care. CERTIFICATION AND CLEARANCE: Medication Technician Certification or State equivalent Requires First Aid and CPR certification; Criminal record clearance or criminal record exemption, as required by law
