About the Role:

As a Registered Nurse, you will be responsible for providing direct patient care to our residents, including administering medications, developing and implementing individualized care plans, and educating patients and families on disease management and prevention. You will also collaborate with our interdisciplinary team to ensure seamless delivery of care and promote a culture of compassion, respect, and excellence.

Responsibilities:

  • Provide high-quality patient care, implementing evidence-based practices and protocols to achieve optimal patient outcomes

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Administer medications and treatments, as prescribed by physicians

  • Conduct patient assessments, identifying needs and providing interventions to address those needs

  • Educate patients and families on disease management, prevention, and wellness strategies

  • Collaborate with the interdisciplinary team to develop and implement care plans

  • Document patient progress and outcomes in accordance with policies and procedures

  • Participate in quality improvement initiatives to continuously improve care delivery
