About the Role

We are seeking a compassionate and dedicated PRN Registered Nurse (RN) to join our in-home care team. This position is ideal for a skilled nurse who values flexibility and enjoys providing personalized, one-on-one care to patients in the comfort of their homes.

Responsibilities

  • Complete patient admissions and start-of-care assessments

  • Conduct supervisory visits to ensure high-quality care delivery

  • Document all visits and assessments in compliance with agency and regulatory requirements

  • Communicate with caregivers, physicians, and the care team to support patient needs

  • Provide education and guidance to patients and their families

  • Deliver compassionate, patient-centered care while promoting independence and safety in the home

Qualifications

  • Active Registered Nurse (RN) license in the state of Georgia

  • Home health experience preferred but not required

  • Strong clinical assessment and documentation skills

  • Ability to work independently and manage time effectively

  • Compassionate, reliable, and patient-focused

Communicates with ASAP Case Managers or client’s legal representative/family any concerns or issues regarding client status as necessary. 7. Performs other duties as assigned. Demonstrated competence in applying processes designed to ensure adherence to all pertinent federal and state rules and regulations. Strong communication skills. Possess the ability to clearly and effectively communicate with staff, physicians, referral sources, patients and their families. Demonstrated computer literacy skills. Qualifications: Required Graduate of an approved school of professional nursing. Current licensed Registered Nurse in practicing state.
