The Caregiver is responsible for providing high-quality, compassionate care to clients in their homes. Responsibilities include assisting with activities of daily living, implementing care plans, promoting comfort and safety, and maintaining accurate documentation.

PRIMARY RESPONSIBILITIES

Assist clients with personal hygiene (bathing, grooming, toileting, etc.)
Support activities of daily living (feeding, mobility, positioning, etc.)
Promote client safety and comfort by following care plans and proper procedures
Maintain confidentiality and uphold agency standards and policies
Document care provided and client responses in a timely manner
Provide companionship and emotional support to clients and families
Work collaboratively with families, supervisors, and care team members
Perform all other duties as assigned

QUALIFICATIONS

High school diploma or GED (preferred)
At least 2 years of related client care experience (preferred)
CNA/HHA/Direct Care Worker certification (preferred)
Valid driver’s license and reliable, insured transportation required
Strong communication and interpersonal skills
Must pass background and reference checks

SHIFT & LOCATION

Every other weekend (Saturday and Sunday)
Every Saturday or Every Sunday
Service areas: Montgomery County (King of Prussia, PA | Hatfield, PA | Royersford, PA)

PHYSICAL REQUIREMENTS

Ability to lift, carry, push, or pull up to 50 lbs. as needed

Summary: The Penn Medicine at Home (PMAH) Home Health Aide is responsible for implementing a Plan of Care, assisting the patient/family with performing personal care tasks, and assisting with the activities of daily living. The individual is responsible for performing home health or hospice visits throughout a designated service area. A Home Health Aide is responsible for completing timely and accurate submission of patient documentation. Responsibilities: Patient Care – Directly performs or assists with bathing, oral hygiene, skin and hair care of the patient. Prepares light meals for patients and assists with eating. Assists with the clothing of patient. Assists the patient with use of a bedpan, commode, and urinal. Turns patients in bed to positions of comfort. Patient Environment - Performs household and other environmental services tasks specified in the Home Health Aide Plan that are essential to entity.
