Job Summary

Cypress Garden Center for Nursing & Rehabilitation is seeking a compassionate and dependable Home Health Aide (HHA) to join our team on a PRN basis. You will provide essential support to residents by assisting with transportation to medical appointments, ensuring safety during outings, and offering a caring presence throughout the day.

Key Responsibilities

Escort residents to and from clinic appointments, dialysis treatments, and other off-site medical visits.
Ensure resident safety during transportation and appointments, monitoring for fall risks or other concerns.
Assist residents with preparation for appointments, including dressing and gathering necessary items.
Provide snacks and support during out-on-pass periods as needed.
Promptly report incidents, accidents, or changes in resident condition to the Clinical Coordinator or ADNS.
Maintain professionalism and represent the facility positively while off-site.
Adhere to all facility policies and procedures at all times.

Minimum Qualifications

Current New York Home Health Aide (HHA) certification required.
Strong communication and interpersonal skills.
Dependable, flexible, and able to work independently.
Compassionate and patient with a strong commitment to quality resident care.
