Responsibilities:

  • Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes

  • Develop, implement, and update individualized Plans of Care

  • Complete OASIS assessments and clinical documentation accurately and on time

  • Monitor patient progress and communicate changes with physicians and the interdisciplinary team

  • Educate patients and caregivers on disease management, medications, treatments, and safety

  • Coordinate care with physicians, therapists, home health aides, and other healthcare professionals

  • Supervise Home Health Aides in accordance with state and federal regulations

  • Promote safe, compassionate, high-quality patient care while helping patients remain safely at home

Qualifications:

  • Graduate of an accredited Registered Nursing (RN) program

  • Current, unrestricted Michigan Registered Nurse license

  • Current CPR certification

  • Minimum of one year of nursing experience required

  • Home Health experience is preferred but not required

  • OASIS experience is a plus

  • Valid driver's license, reliable transportation, and auto insurance
