As we continue to grow, we are seeking an experienced and compassionate Registered Nurse (RN) who shares our passion for supporting seniors in the home setting and values team collaboration, care coordination, and high-quality patient care.

Position Summary

The Part-Time RN plays a key clinical leadership role in supporting our clients through assessments, care planning, supervisory visits, and skilled nursing services as needed. This role requires strong clinical judgment, attention to detail, and a genuine commitment to patient-centered care.

Key Responsibilities

  • Conduct initial client assessments and ongoing reassessments

  • Develop and update personalized Plans of Care

  • Perform caregiver supervisory visits to ensure quality and compliance

  • Provide skilled nursing visits as ordered (when applicable)

  • Provide caregiver education and skills training to ensure safe, high-quality care delivery

  • Ensure compliance with New Jersey state regulations and agency standards

  • Complete accurate and timely documentation

  • Support case management and care coordination

  • Collaborate with caregivers, families, and healthcare partners to ensure continuity of care

  • Contribute to building and maintaining positive relationships within the community

Qualifications

  • Active New Jersey RN License (required)

  • Minimum 2+ years of clinical experience

  • Prior experience in home care or home health preferred

  • Strong patient assessment and care planning skills

  • Knowledge of state healthcare regulations and compliance standards

  • Experience in caregiver training, coaching, or supervision is a plus

  • Excellent documentation and communication skills

  • Ability to work independently and as part of a team

  • Reliable transportation required

  • Weekend and evening availability a plus

Compensation & Benefits

  • Competitive pay: $35–$40/hour (based on experience)
