Job Title: 1:1 Registered Nurse (School-Based)
Location: Zimmerman, MN
Schedule: 4 Hours/Day (7:45 AM 11:45 AM), School Days
Dates: ASAP June 4, 2026 (Current School Year + Summer)
Renewal: Likely to continue into the next school year (NSY) based on fit

Position Overview:
We are seeking a highly skilled and compassionate Registered Nurse for a 1:1 school-based position supporting a medically complex student returning to school on a part-time basis. The students day will begin at approximately 4 hours, with plans to gradually increase as stamina improves and transition toward full-time attendance.

Key Responsibilities:

  • Provide direct 1:1 nursing care throughout the school day
  • Ride transportation to pick up the student from home, accompany them to school, and return home (all time is paid)
  • Monitor the students overall health status and respond to changes
  • Administer tube feedings and medications
  • Manage tracheostomy care and ventilator support
  • Monitor continuous glucose levels and respond to low blood sugar related to adrenal insufficiency
  • Recognize and respond to seizure activity and implement emergency care plans as needed
  • Collaborate closely with the Licensed School Nurse (LSN), school staff, and family
  • Maintain accurate and compliant documentation

Student Care Needs:

  • Adrenal insufficiency with risk of low blood sugar (uses a continuous glucose monitor)
  • Feeding tube care
  • Tracheostomy with ventilator support
  • Seizure management

Support & Environment:

  • A paraprofessional will assist the nurse with diapering needs
  • Student requires a 2-person transfer using a mechanical lift; an assistant will support all transfers alongside the nurse
  • Nurse will begin and end their day at the elementary school, including all bus transportation responsibilities

Qualifications:

  • Active Registered Nurse (RN) license in Minnesota
  • Experience with tracheostomy/ventilator care, tube feeding, and/or pediatric complex care preferred

PURPOSE AND SCOPE: Manages patient care in home therapies programs while maintaining cost-effective clinical operations.
