Full time, 3pm-11pm. Every Sunday, Monday, Friday, and Saturday.  Holiday rotation.    

Our Inpatient Unit on the Mitchell Campus in Cheektowaga, NY, provides a relaxing home-like setting for select patients and their families. Our unique unit is only offered to patients whose pain or symptoms cannot be managed at home or are approaching their final stage. Hospice patients can also be admitted to the unit for short-term respite care.

A Hospice Inpatient Unit Registered Nurse renders professional nursing services, in accordance with physicians' orders, for individualized patient care and psychosocial/spiritual interventions for assigned patients/families. Provides nursing care in accordance with Hospice objectives and follows policies and procedures of the organization and the State Nurse Practice Act.

Essential Duties and Responsibilities for a Hospice Inpatient Unit Registered Nurse:

Demonstrates ability to perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings on all appropriate forms and on the plan of care.

Performs pain assessments using Edmonton Scale, adhering to pain protocols, knowledgeable in equi-analgesic dosing and assures optimal pain relief using an interdisciplinary focus and timely appropriate interventions.

Organizes and prioritizes assignments to consistently meet productivity standards and complete documentation requirements within identified time parameters.

Appropriately explains duties and functions of the interdisciplinary team; articulates disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, and other neurologic disease states.

Able to articulate etiologies of symptom manifestation (nausea, vomiting, anorexia, fluid and electrolyte imbalance, diarrhea, constipation, mucositis, weakness/fatigue, dysphasia, edema, dyspnea, cognitive impairment, urinary retention, depression, delirium) related to a disease process.

Modifies the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control.

Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, and diuretics.

Documents to agency standards, timely completion, timely signatures, and documents care using A-I-R protocol to support hospice stay in program and level of care.

Elicited no founded patient/family complaints or unusual occurrences (i.e. reportable incidents).

Demonstrates proper sharps disposal, hand hygiene, waste disposal, use of personal protective equipment.

Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative); Adheres to HIPPA regulations and policies.

Maintains the security of patient electronic/written records.

Incorporates computer documentation into patient visit and/or as much as appropriately possible without compromising the quality of care and/or communication.

Assures the timely completion and return of all initial, renewal and modified orders and reviews all PPOC's for procedural compliance.

Qualifications for a Hospice Inpatient Unit Registered Nurse:

Education/Certification

Graduate of an accredited school of nursing with current RN licensure in the State of New York.

Certification in Hospice and Palliative Care Nursing preferred.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience

Year of nursing practice as a professional nurse is preferred.

Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning delivery of care. 2. Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, TPN, infectious disease and wound care, if applicable. 3. Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained. 4. Attends all mandatory inservices by employee’s anniversary date. 5. Maintains current New York State license in compliance with New York State regulations. 6. Works with interdisciplinary team members in developing a discharge plan for individual residents and assists in achieving discharge planning. 7. Interprets and implements principles of good nursing to nursing staff. 8. Evaluates incidents and determines emergency measures to be used, including the use of restraints. Assesses and directs emergency treatment to residents and staff involved in accidents. If necessary, notifies the attending physicians. Completes the incident report with causes investigated and corrective actions taken and/or suitable recommendations made to avoid recurrence. Updates the care plan with new interventions. 9. Assists in orienting and directing new staff. 10. Reviews and interprets work performance against accepted standards. May provide input to the Inservice Coordinator in planning nursing inservice programs and on-the-unit conferences based upon an assessment of staff’s learning needs and/or resident care problems. 14. Handles resident or staff grievances and/or refers them to the appropriate department supervisor. 15. Assists in establishing and maintaining a positive staff and good public relations. 16. Maintains accurate, adequate records of resident care and related statistics as required. 17. Is available for emergencies and assistance to nursing staff as needed. 18.
