RESPONSIBILTIES:
Admissions Coordination:Serve as the primary point of contact for new admissions/readmissions, ensuring all aspects of the process are handled professionally and efficiently. Coordinate pre-admission assessments/evaluations with the interdisciplinary team to evaluate potential neighbors' medical and nursing care needs. Communicate effectively with the admissions team and the transferring facility (hospital, community or other care setting).
Clinical evaluation: Conduct thorough evaluations of incoming neighbors, including physical, mental, and emotional health evaluations. Review medical histories, medications, and provider’s orders to develop initial care plans. Communicate and document any special needs or considerations for each resident. Review and support neighbors and family in any advanced directive care needs
Documentation and Compliance: Complete and ensure accuracy of all admission paperwork, including discharge summaries, admission checklists consent forms, medical records, and regulatory documentation. Maintain compliance with state and federal regulations, including Medicare/Medicaid requirements and HIPAA standards. Update the electronic health record (EHR) system with all relevant information promptly. Ensure an active transition of the neighbors entering the facility from another care setting.
Resident and Family Support: Welcome neighbors and their families upon admission/re-admission, providing a thorough orientation to the facility and its services. Act as a liaison between residents, families, and the healthcare team, addressing questions and concerns with empathy and professionalism. Provide education to residents and families about care plans, medications, and facility policies. Collaborate with unit-based staff to ensure room readiness
Collaboration and Communication: Work closely with the admissions team, nursing staff, and other departments to coordinate the arrival and placement of new/returning neighbors. Collaborate with external healthcare providers, hospitals, and referral sources to ensure continuity of care.
Quality Assurance: Monitor and evaluate the admissions process, identifying areas for improvement and implementing solutions. Ensure that neighbors’ initial care plans align with their specific needs and that care delivery meets the facility’s standards.
Other Duties: Act in a nursing supervisor capacity when needed. Perform bedside nursing duties such as medication and treatment administration, wound care as well as documentation of care and C.N.A. oversite when needed at the discretion of the director of nursing services, assistant director of nursing services or the nursing supervisor. The admission nurse is expected to participate in the manager on duty rotation as are all nurses in leadership positions.
REQUIREMENTS:
Education and Experience:
Required: Nursing license in New York State, in good standing.
Preferred: Clinical and supervisory experience in a long-term or sub-acute setting.
Able to make decisions with good nursing judgement, handle emergencies, and maintain excellent patient evaluation skills. Act as an administrative representative of the facility.
