As a Household Coordinator with Heritage House, your responsibilities will include:

  • Set an example of encouraging resident choice and providing a social model of care.
  • Provide leadership as a member of the Nursing Leadership Team
  • Perform all of the duties of a nurse for the purpose of providing health care services to residents and their families.
  • Assist is the evaluation of the health care services needed for each resident relative to the level of lifestyle accommodation. 
  • Participate in ensuring the community is following all applicable regulations at all times.
  • Maintain confidential relations with residents and families while assisting in providing spiritual, psychological, and social needs of residents
  • Participates in on-going Quality Assurance programming and conducts audits as requested
  • Assists in documentation for MDS data collection

Qualifications: 

  • Currently licensed LPN in the State of Iowa
  • Prior clinical management experience required, prior long term care experience strongly preferred

Complies with all company safety policies and practices. Assists in relevant health and psychiatric teaching for client and family. Serves as a role model for clients and peers. Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care. Effectively communicates with the medical staff, receives verbal/written orders, and takes care to minimize abbreviation or other errors. Participates in team conferences, sharing knowledge, information, and ideas among team members. Prepares, administers, controls and accurately records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse, with thorough documentation regarding administration effect and/or adverse effects.
