RN Registered Nurse Case Manage Job Summary:

We are seeking a dedicated and compassionate RN Registered Nurse Case Manager to join our team at Genesee Senior Living in Batavia, NY. As a Registered Nurse Case Manager, you will play a vital role in ensuring our residents receive the highest level of care and support. This is a full-time, Monday-to-Friday position that offers a dynamic and rewarding work environment.

RN Registered Nurse Case Manage Responsibilities:

  • Coordinate and manage care for residents across our assisted living facility, ensuring that their needs are met and exceeded

  • Develop and implement individualized care plans, working closely with interdisciplinary teams to achieve optimal outcomes

  • Provide direct patient care, including assessments, medication management, and wound care

  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless transitions and optimal care

  • Consult with families and caregivers to educate and support them in their loved one's care

  • Participate in quality improvement initiatives to ensuring continuous quality and safety

LPN/RN Nurse Case Manager Requirements:

  • Current RN license in the state of New York

  • Prior experience in a healthcare setting, preferably in a case management role

  • Strong assessment, communication, and problem-solving skills

  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks

  • Strong attention to detail and organizational skills
