Now Hiring: Wellness Coordinator

The Wellness Coordinator plays a key role as a member of the comprehensive interdisciplinary healthcare team that works closely with individuals, families, and others. Your main goal is to use all of your training to advocate for and provide person-centered care to community members. Planning, organizing, developing, facilitating, and directing activities that people find interesting is the main goal of your job in order to give them meaningful ways to spend their time, such as having fun, exercising, and fostering relationships.

Responsibilities:

• Manage the coordination of care for residents, including the dissemination of information to families, staff, physicians, and third-party providers.
• Assist the Executive Director to ensure that policies, procedures, and state regulations are being followed.
• Cover shifts as needed, assisting residents with cleaning, bathing, dressing, eating, personal care, and hygiene.
• Communicate with other healthcare staff and department personnel; relate information concerning resident’s condition; maintain written records and documentation as required.
• Maintain communication with the residents, their families, support personnel, etc., to assure that the resident's needs are continually met. health care, independent living, active, events, patient care, dementia, caregiver, immediately hiring, nursing home, aide, assisted living, memory care, hiring immediately, senior living.

Qualifications:

• LVN
• Minimum two years experience in assisted/senior living or long-term care.
• Demonstrated good judgment, problem solving and decision making skills.
• Proficient computer skills, including Microsoft Office with the ability to learn new applications.
• Effective organization, time management, and written and verbal communication skills.
• The ability to handle multiple priorities and delegate assignments
• A dedication to seniors and their well-being is also essential
