MDS Coordinator (RN/LPN)

We are seeking a detail-oriented and compassionate MDS Coordinator to join our clinical leadership team.
Position Summary
The MDS Coordinator is responsible for overseeing and coordinating the Resident Assessment Instrument (RAI) process to ensure timely and accurate completion of MDS assessments in accordance with Medicare, Medicaid, OBRA, and Managed Care requirements.
This role plays a critical part in supporting clinical excellence, regulatory compliance, and appropriate reimbursement, while ensuring each resident receives individualized, high-quality care.
Qualifications:
Active RN or LPN license in the state of Connecticut
Minimum of 1 year of experience with MDS, PDPM, and OBRA assessments
Strong understanding of Medicare and Managed Care documentation requirements
Knowledge of reimbursement systems and regulatory compliance
Excellent organizational, communication, and analytical skills
Commitment to compassionate, resident-centered care
Key Responsibilities:
Coordinate and complete the MDS/RAI process in compliance with federal and state regulations
Perform initial and ongoing resident assessments to support individualized care planning
Develop and update comprehensive care plans in collaboration with the interdisciplinary team
Ensure accuracy, completeness, and timeliness of all MDS submissions
Monitor clinical documentation to support appropriate reimbursement under PDPM
Partner with nursing, therapy, and administrative teams to drive quality outcomes
Track assessment schedules and maintain compliance with required timelines
Participate in care plan meetings and effectively communicate resident needs and goals
Stay current with regulatory changes and best practices to ensure ongoing compliance
Participate in Manager on Duty (MOD) rotation as part of the leadership team, with no nursing on-call requirement
