The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families

Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers

Performs comprehensive patient examinations, evaluations, assessments and diagnosis

•

Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs

Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients

•

Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested

•

Performs timely face to face assessments with patients

•

Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care

Completes all documentation in an accurate, legible and timely manner

Job Description The Division of Neuro-Oncology within the Department of Neurosurgery is a referral, recommendation, and treatment service for adult patients with primary and metastatic brain tumors. The Neuro-Oncology Service Clinical Trials Program is an active research program that includes studies supported by the Pharmaceutical Industry; investigator initiated studies, studies in collaboration with outside universities and NCI-cooperative groups; and SPORE grants for cancer research. The Division of Neuro-Oncology as a part of UCSF is committed to providing outstanding care to our patients, and includes clinicians with experience and expertise working with patients of diverse cultures and experiences. Led by Edward Chang, MD, Chair of the Department of Neurological Surgery and John De Groot, MD Director of the Division of Neuro-Oncology the program is staffed by a strong group of collegial faculty, with Medical, Nursing, Neuropsychological staff and Research staff contributing to a high-quality collaborative multidisciplinary team. To support the growth within our division, we are now seeking to recruit another advanced practice provider. The APP will provide pre-appointment screening, clinical assessment, treatment, and disposition for inpatients and outpatients patients with primary brain tumors. The APP will also work collaboratively with our Neuro-Oncologist and Neuropsychologists for rehab planning and care management. Clinical services will be provided in accordance with UCSF-approved Standardized Procedures, and under the supervision of UCSF Neuro-Oncology physicians. Work schedule is full-time and working hours will be determined based on patient care need schedule. The APP works with the neuropsychologist/physicians/surgeons/AHP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, performing histories and physicals, performing quality of life assessments, responding to change in patient status, providing needed prescriptions/evaluations/referral/treatments as permitted under their standardized procedures. Home health services and DME, DMV evaluations, referrals, consultations, radiographic or diagnostic requests, specialized treatment privileges, providing patient education, arranging follow-up appointments, and engaging in any approved standardized procedures require that the AHP has OPPE/FPPE processes completed through the MSSO prior to independent patient care. The APP will also play a pivotal role in the development and dissemination of key programmatic resources. This will include the organization of community outreach efforts and patent educational materials, as well as participation in quality improvement symposia and executive leadership meetings.
