Responsibilities

The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families

Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers

Performs comprehensive patient examinations, evaluations, assessments and diagnosis

•

Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs

•

Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients

•

Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested

•

Performs timely face to face assessments with patients

•

Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care

•

Completes all documentation in an accurate, legible and timely manner

•

Provides quality patient care consistent with standards of nursing practices

•

Complies with hospice policies and procedures, laws and regulations and standards of practice

Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. The APP will be responsible for coordinating the care of patients in the hospital setting Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege).
