About the Role:

As a Per Diem Registered Nurse (RN), you will play a vital role in delivering high-quality care to our patients in their homes and communities. You will conduct in-home or community-based assessments, perform head-to-toe evaluations, complete aide supervisions, and provide clinical guidance. You will also develop and update personalized Plans of Care (POC), provide orientation to aides based on the POC, and accurately document findings to maintain compliance with healthcare regulations.

Responsibilities:

• Conduct in-home or community-based assessments
• Perform head-to-toe evaluations, including medication and diagnosis review
• Complete aide supervisions and provide clinical guidance
• Develop and update personalized Plans of Care (POC)
• Provide orientation to aides based on the POC
• Accurately document findings and maintain compliance with healthcare regulations

Requirements:

• Current New York State RN license
• Ability to travel throughout the Albany area
• Medical Professional Liability insurance
• Comfortable using digital documentation tools

Preferred Qualifications:

• Bilingual in English
• Familiarity with Medicaid, Medicare, or MLTC plans
• Experience in outpatient nursing, home care, or community health
• Strong documentation and communication skills
