Job Summary:
As an LPN/RN MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining compliance with Medicare and Medicaid regulations. You will work closely with our care team to develop and implement individualized care plans that meet the unique needs of our residents.
Responsibilities:
Coordinate the MDS assessment process, including scheduling and conducting assessments, and reviewing assessments for accuracy and completeness
Ensure timely completion of MDS assessments and communicate with families and healthcare providers as necessary
Develop and implement individualized care plans that meet the unique needs of our residents
Collaborate with the care team to identify and address resident needs and concerns
Maintain accurate and up-to-date records and documentation
Participate in quality improvement initiatives to ensure compliance with Medicare and Medicaid regulations
Provide education and support to families and caregivers on care planning and resident changes
Requirements:
Current LPN or RN licensure in the state of Missouri
2+ years of experience in long-term care, rehabilitation, or acute care setting
Strong knowledge of Medicare and Medicaid regulations and guidelines
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment with multiple priorities
Strong organizational and time management skills
