About the Opportunity

As a Home Care Field RN / Outpatient Nurse, you will travel to client homes and community-based settings across assigned counties, conducting in-home or community-based nursing assessments, and providing high-quality care to patients in their homes and communities. Our nurse's play an important role in supporting patients, families, and caregivers, while helping ensure each client receives care that is safe, personalized, and respectful.

Key Responsibilities:

• Travel to client homes and community-based settings across assigned counties

• Conduct in-home or community-based nursing assessments

• Perform head-to-toe evaluations, including medication and diagnosis review

• Complete aide supervisions and provide clinical guidance

• Develop, review, and update personalized Plans of Care (POC)

• Provide orientation to aides based on the client's Plan of Care

• Monitor client condition and report any changes as needed

• Communicate with patients, families, caregivers, and office staff

• Accurately document findings and maintain compliance with healthcare regulations

• Help ensure quality care is being delivered according to each client's needs

Requirements:

• Current New York State RN license

Per Diem Registered Nurse

Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Job Description Job Responsibilities/Essential Functions Utilizes the nursing process effectively (assessing the patient and situation, identifying the problem, planning, implementing, and evaluating care through the health care continuum). Triages phone calls utilizing critical thinking and assessment skills to evaluate the chief complaint and urgency of the problem. Triages phone calls to determine acuity to assist in scheduling appointments. Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality. Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues.
