Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager.

RECRUITMENT REQUIREMENTS:

 

High School diploma or equivalent preferred;  Must be able to read and understand written instructions; Must be able to take direction via telephone in patient’s home; Must be able to make meaningful observations and report changes in patient condition; Must have basic knowledge of computer applications and ability to enter patient information into an electronic device. Successfully pass all competency exams.

 

LICENSE AND REGULATORY REQUIREMENTS:

 

Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver’s license and access to a car, current car registration, and current car insurance.
