The MDS Coordinator is a key member of the Jacksonville Leadership Team who will help integrate and coordinate a resident-centered nursing strategy with a strong focus on quality care and accurate clinical assessments. The primary focus of this role is overseeing the resident assessment process, including MDS assessments, individualized care planning, and coordination of care throughout the resident's stay.

Your Role:

  • Provide accurate documentation to ensure proper reimbursement from all payor sources.

  • Coordinate and complete MDS assessments in a timely and accurate manner.

  • Develop, review, and revise individualized resident care plans as appropriate.

  • Ensure residents receive the resources and support needed for quality care delivery.

  • Collaborate with interdisciplinary team members to support resident outcomes.

  • Maintain compliance with long-term care regulations and documentation standards.

  • Participate in care conferences and communicate effectively with residents and families.

  • Assist with additional clinical and administrative duties as assigned.

Who You Are:

  • Current RN or LPN license in the state of Illinois.

  • Long-term care experience required.

  • 1–2 years of MDS experience preferred.

  • Knowledgeable in long-term care nursing practices, regulations, and documentation standards.
