SUMMARY: SUMMARY: Under the direction of leadership, the Nurse Navigator provides individualized clinical care coordination for patients with complex, chronic, or specialty specific healthcare needs across the continuum of care. Working collaboratively with patients, families, providers, and interdisciplinary teams, the Nurse Navigator identifies barriers to care, develops patient-centered navigations plans, coordinates services, promotes self-management, and facilitates timely access to appropriate care to improve clinical outcomes and patient experience. This position supports multiple service lines using standardized navigation processes while applying specialty-specific clinical knowledge as assigned. Essential Functions include clinical assessment and care planning, care coordination, patient education and advocacy, interdisciplinary collaboration, documentation and quality improvement.
EXPERIENCE: Minimum of three (3) years clinical experience as a licensed, registered nurse, two (2) years, care coordination, case management, ambulatory nursing, disease management or patient navigation experience preferred.
SPECIAL SKILLS: Strong clinical assessment and critical thinking skills. Knowledge of care coordination, population health, and transitions of care. Excellent communication, collaboration, and patient education skills. Ability to prioritize multiple patients while maintaining quality and service standards. Proficiency with EMR and Microsoft Office applications.
LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri, BLS (Basic Life Support) from AHA (American Heart Association) Helpful: Certification in Specialty Area
EDUCATION: Required: Bachelors - Nursing, Preferred: Masters - Nursing
