Position Summary

The Clinical RN is a professional caregiver who assumes responsibilities and accounts for a group of patients for a designated time frame and provides therapeutic and safe care to patients using the nursing process of individualized patient/client assessment, outcomes identification planning, implementation/intervention and evaluation. The RN is responsible and accountable for nursing care delivered to their assigned patients by LVNs, HAs and others. Professional duties include patient teaching, staff development, delegation, supervision, coordination, and patient advocacy. The RN will demonstrate competency and commitment in achieving organizational and nursing department goals, as outlined in the RN Behavior List and Competency Checklist.



  • Essential Duties And Responsibilities

    • Complies with CHA Compliance Handbook including Code of Ethics and all Statues, regulations and guidelines applicable to federal and state programs.     Qualifications

    Required

        • Currently licensed in the State of California to practice as a Registered
        • Six (6) months previous experience in Medical-Surgical (M/S) nursing
        • Basic Cardiac Life Support (BCLS) Certification
        • Able to speak, write and understand English
        • Bilingual Cantonese-speaking preferred
        • Familiarity with computer
    Specific Qualification For Specialty Units

    • Advanced Cardiac Life Support (ACLS) certificate required for ICU, Emergency department, Post Anesthesia Care Unit (PACU), Same Day Surgery Unit (SDSU) and Endoscopy Suite (ENDO). CCRN recommended for ICU. Current Oncology Nursing Society (ONS) certification required for core Outpatient Medical Therapy Center (OMTC) staff.
    • Must be able to assess and care for the special needs and behavior ofspecific age population:
    • Medical/Surgical (M/S), Telemetry Units and ICU – Adolescent to frail elderly
    • Surgical Suite/PACU – Adolescent to frail elderly
    • SDSU/ENDO Suite/OMTC – 13 years to frail elderly

    Base Pay Scale

    Starting at $61.87 -$91.85 per hour. The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.

We place a high importance on our employees and reward staff in several ways such as: • Competitive hospice industry compensation • Benefits package with multiple plan offerings and generous employer contribution • 401(k) Retirement plan with employer match • Supportive work culture which encourages work life balance • Substantial Paid Time Off, paid holidays & birthday benefit • Employee development program managed by in-house education department to support individualized orientation and professional growth needs. • Monthly mileage reimbursement • Employee Referral Program • Business casual attire for office staff Responsibilities JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE: • Develops written plan of care in coordination with an interdisciplinary Team (IDT). • Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs. • Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care. • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements. • Communicates all changes to IDT and primary MD. • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice. • Maintains timely patient documentation using “point care” electronic charting. • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements. • Provides health care instruction to patient and family members. • Orients, instructs, and trains assigned personnel. • Participates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation). • Performs admission assessment and admission when necessary. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
