Job Summary:
SUNY Upstate is seeking a highly motivated Retention and Adherence LPN Specialist to provide services to patients seen in the Inclusive Health Services and the Pediatric Infectious Disease Clinic's grant funded retention and adherence program.
Position provides linkage to and retention in medical care as well as Intensive Case Management to a specific cohort of patients, those who are new to antiretroviral therapy and/or who have not achieved viral suppression.
Primary Responsibilities:
Assist patients in identifying the barriers preventing them from engaging in medical care and from attaining a suppressed HIV viral load.
Work with the patients to create a plan to address these barriers.
Encourage patients in reaching their goals and follow up with them as needed.
Services will improve the health outcomes for the patients and decreasing the community viral load through prevention for positives.
Must have strong interpersonal, written and oral communication skills.
Must be able to work in an interdisciplinary team environment.
As a Retention and Adherence LPN Specialist, you will be an integral member of the Healthcare Team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
Current NYS LPN license with a minimum 5 years experience in Health and Human Services.
CPR Certification required at the time of appointment.
Case Management Experience.
Experience in field of HIV/AIDs.
Preferred Qualifications:
Must possess strong interpersonal, written and oral communication skills.
Must be able to work in an interdisciplinary team environment.
Work Days:
Monday-Friday 8am-4:30pm
