At Phoenix Hospice, our Admissions RNs are the first calm presence when hospice care begins. You’ll guide patients and families from curative care into comfort-focused care with compassion, clinical excellence, and dignity. This role blends strong clinical judgment with deep human connection—addressing not only physical needs, but also emotional, spiritual, and psychosocial support.

You’ll collaborate closely with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure every admission is smooth, thoughtful, and centered on what matters most.

What You’ll Do
  • Complete hospice admissions and skilled nursing visits
  • Build meaningful, day-to-day connections with patients and families
  • Provide education on conditions such as CHF, COPD, and Diabetes
  • Perform wound care and medication management
  • Change dressings and medical devices as ordered
  • Accurately document patient conditions and communicate changes to physicians
  • Partner closely with interdisciplinary hospice teams to deliver seamless care

What You’ll Need
  • Hospice experience required
  • Active Missouri or Compact RN license
  • Strong assessment, organization, and communication skills
  • Ability to work independently while collaborating with a team
  • Valid driver’s license
