As a Registered Nurse at MercyOne, you will provide and direct safe, effective, and culturally competent care for patients with acute medical and surgical conditions. Through evidence-based practice, utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care. The Acute Care RN manages a diverse patient population with varying levels of acuity, including post-operative, medical, telemetry, and neurologic patients. Supervises and coordinates care provided by LPNs, Patient Care Assistants, and other nursing team members. Facilitates communication with physicians, patients and families, and interdisciplinary team members. Adheres to MercyOne’s performance expectations and upholds the Mission, Vision, and Values of MercyOne.

Essential Functions:

  • Conducts assessment

  • Identifies patient problems and develops plan of care

  • Implements outcome/goal - based plan of care

  • Evaluates patient progress and revises the outcome/goal - based plan as needed

  • Communicates relevant clinical information to the team to optimize patient outcomes/goals

  • Demonstrates clinical competence

  • Manages telemetry and cardiac monitoring when applicable

  • Provides care appropriate to the age-related needs of the patients served on his/her assigned unit

  • Adheres to all safety, infection control and colleague health policies and procedures

  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

Minimum Qualifications:

  • Associate degree in nursing required

  • Bachelor of Science in Nursing (BSN) preferred

  • Registered Nurse in Iowa, Registered Nurse in Illinois within 1 year

  • American Heart Association Basic Life Support (BLS) required

  • American Heart Association Advanced Cardiac Life Support (ACLS) required only if the unit has telemetry monitoring capabilities

  • Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire

MDS Coordinator

MDS Coordinator Job Summary: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix and care plan documentation in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) and care plan reviews to assure completeness, thoroughness, and achievement of the optimal allowable Patient Driven Payment Model (PDPM) and Resource Utilization Group (RUG) categorization. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission, care plan documentation, and the restorative nursing program. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. Essential Duties: · MDS Assessment and Documentation · Assesses and determines the health status and level of care of all new admissions. · Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. · Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. · Care Planning and Resident Assessment · Completes care plans on admission, quarterly and as needed for each resident according to regulatory time frames.
