Responsibilities
  • Provide clinical oversight and nursing supervision during assigned shifts in accordance with resident care plans, physician/provider orders, facility policies, and applicable standards of nursing practice.
  • Assess and monitor residents' health status, identify changes in condition, initiate appropriate nursing interventions, and ensure significant findings are promptly communicated to the appropriate provider and members of the interdisciplinary care team.
  • Provide direction, supervision, and clinical support to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) within their respective scopes of practice and assigned responsibilities.
  • Delegate nursing tasks appropriately and monitor the performance and completion of delegated care in accordance with applicable requirements and facility policy.
  • Review and support the delivery of resident care to promote compliance with individualized plans of care, physician/provider orders, and established clinical standards.
  • Administer medications, treatments, and nursing procedures as required and within the RN scope of practice.
  • Respond to changes in resident condition and clinical emergencies, initiating appropriate interventions and coordinating additional medical care when necessary.
  • Communicate and collaborate with physicians, advanced practice providers, nursing staff, rehabilitation professionals, and other members of the interdisciplinary care team to support coordinated resident care.
  • Maintain accurate, timely, and complete clinical documentation in the facility's electronic or paper medical record.
  • Support appropriate shift communication and continuity of care, including reporting relevant resident concerns and clinical information to incoming nursing staff.
  • Promote adherence to infection prevention and control practices, resident safety requirements, and established facility procedures.
  • Provide clinical guidance and support to nursing staff while promoting professionalism, accountability, teamwork, and high standards of resident care.
  • Promote resident dignity, comfort, safety, rights, and independence while maintaining positive and professional relationships with residents, families, facility staff, and other healthcare professionals.
  • Adhere to all applicable New York State and federal requirements, professional standards of nursing practice, facility policies, infection control standards, and resident confidentiality requirements.

Required Qualifications:

  • Current New York State Registered Nurse license
  • Recent experience working as an RN in a facility setting (such as assisted living, rehab facility, nursing home, or adult day care)
  • Experience administering medications and treatments per provider orders in a facility setting
  • Experience monitoring, assessing, and documenting vital signs and patient status in a facility setting
  • Experience collaborating with facility staff and reporting patient condition changes
  • English proficiency for patient and staff communication
  • Authorized to work in the United States
  • Reliable transportation and ability to travel up to 25 miles between assigned facilities

Preferred Qualifications:

  • Experience with facility EMR/documentation/charting systems
  • BLS/CPR certification
