You will have responsibilities including but not limited to:

Conduct a timely comprehensive assessment of acute psychiatric and substance use disorder related conditions.

Maintain an individual case panel providing ongoing psychiatric, and substance use disorder assessment, medication management, treatment planning, and follow up.

Provide cross-coverage for clients assigned to other providers as needed for continuity of care, and timely access to care.

Assess and respond to acute clinical needs including but not limited to suicidal or homicidal ideation, self-harm risk, mania, psychosis, severe anxiety, withdrawal, intoxication, substance cravings or significant changes to mental status or presentation within the scope of care practice while engaging with on-call medical director, nursing leadership and other supports.

Review past medical, psychiatric and substance use history of new admissions and assess for urgent/emergent conditions that need immediate attention.

Maintain thorough and accurate and timely documentation of clinical encounters including clinical decision making. Completion of notes within 48 hours of client encounter – sooner based on acuity.

Document all encounters with client and or staff related to client conditions or concerns in the medical record.

Participates in interdisciplinary care and case conferencing, working closely with nursing, counselors/CASACs, licensed mental health clinicians, recovery support specialists (RSS), peers, medical providers, and other team members.

Coordinate with pharmacy, medical team, and outside providers as needed to promote continuity of care.

Provide on call weekend and holiday coverage as assigned with timely responses to clinical questions during coverage.

May be asked to provide emergency/urgent coverage at alternative Phoenix House locations based on need either in person or virtually based on organizational or client need.

Ensure timely follow up for clients seen in the ED or admitted to the hospital to promote safe transitions of care.

The Psychiatrist will conduct examinations on patients scheduled by the institution's medical staff. Responsibilities include conducting psychiatric evaluations and making referral recommendations for inmates. Each workday will consist of eight (8) sessions, totaling two (2) workdays per week (16 sessions + unpaid half-hour lunch). The candidate will have eight (8) sessions to complete a workday. Clinics will typically be scheduled between the hours of 7:30 am and 4:00 pm (including an unpaid ½ hour lunch) on Tuesdays and Thursdays, excluding federal holidays. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. .
