The ideal candidate will assist with conducting comprehensive assessments of residents in a long-term care facility using the Minimum Data Set (MDS) system, while also managing the admissions process by evaluating potential residents' needs and coordinating their intake into the facility, ensuring compliance with all regulations and facilitating smooth transitions into care.


Key Responsibilities:

  • Conduct thorough MDS assessments on all new and current residents to accurately evaluate their functional abilities, medical needs, and care requirements.

  • Utilize the MDS system to document resident information and identify potential care concerns.

  • Analyze MDS data to develop individualized care plans with specific goals and interventions.

  • Monitor resident progress and update MDS assessments as needed to reflect changes in condition.

  • Review potential resident applications and medical records to determine suitability for the facility.

  • Conduct initial assessments with prospective residents and their families to discuss care needs and expectations.

  • Collaborate with the admissions team to schedule tours and answer questions.

  • Coordinate the admission process, including paperwork, medical records transfer, and room assignment.

     

Required Skills and Qualifications:

  • Registered Nurse (RN) with current practice credentials in the State of Minnesota 

  • Thorough understanding of the MDS assessment process and Medicare/Medicaid regulations

  • Excellent clinical assessment skills and ability to interpret resident needs

  • Strong communication and interpersonal skills to interact with residents, families, and healthcare team members

  • Detail-oriented with strong organizational skills to manage complex documentation and reporting requirements.

  • Ability to work independently and as part of a collaborative team
