You’ll work full-time, providing direct care and support to a 20-year-old student with specialized medical needs, including tracheostomy and G-tube management, and also assist with daily transportation.

Key Qualifications:

Active Registered Nurse (RN) license required
Previous experience in school nursing or with pediatric/transition-age populations beneficial
Skilled in tracheostomy and G-tube care
Comfortable with wheelchair transfers and mobility support
Demonstrated ability to respond to emergency medical situations with composure
Excellent communication and interpersonal skills to collaborate with educators, therapists, and families
No PEL (Professional Educator License) required for this role

Role Responsibilities:

Provide continuous 1:1 nursing care for a student throughout the school day and during daily bus transportation
Monitor and manage the student’s trach and G-tube, ensuring safety and comfort at all times
Work collaboratively with school staff to support the student’s academic and social engagement
Document daily care, interventions, and any incidents as required by nursing and school protocols
Serve as a compassionate, trusted presence for the student and as a key liaison for families and staff

The CPC communicates the condition of the resident to the family, coordinates the completion of all Comprehensive Resident Assessments and reports to the Administrator. QUALIFICATIONS: A current State of Illinois license as a Registered Nurse or Licensed Practical Nurse. Must be able to read, write, and speak English in an understandable manner. AANAC certification preferred. JOB REQUIREMENTS: Must be physically and mentally capable of performing routine job duties. Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing, the MDS, and it’s computer application (EMR). Must have good managerial, organizational, planning, computer, and interpersonal skills. Ability to initiate, complete and update care plans. Communicate resident care plan and progress to staff, resident and family. Conduct in-service training of nursing staff every three months as needed on care plans and documentation. Audit sample EMR charting monthly to ascertain proper nursing charting and documentation of care given and directly confer with those employees who are not properly documenting. Record, report and follow up discrepancies in documentation which are not consistent and/or do not meet program standards. Work closely with the Director of Nursing and other department heads to ensure consistency in care. Coordinate resident Care Plan with physical, occupational, respiratory, and speech therapy department to ensure proper care and treatment when clinically appropriate. Follow up on nutritional assessments by the dietary consultants with reporting to the nursing staff and appropriate documentation.
