Inputs or assists with ordering all imaging, laboratory, surgery, procedural, and referral needs within the patient medical record using standing orders. Serves as primary role for completion of these orders and follow through for results or medical records for closure. Contacts the pateint/caregiver/family for all results adn provides further education as directed by the ordering/overseeing provider. Documents all pateint interaction efficiently within the electronic medical record. Performs telephone symptom triage and assessment. Uses standing orders and approved protocols to education and guide patients based on assessments. Uses standing orders and protocols to review pateint results and provide instruction under the direction of a phshcian or advanced practitioner. The LPN will be resonsible for reviewing all incoming results for urgency and abnormality and escalate the provider for immediate attention. Collaborates with care team to ensure smooth and effective transitions of care Prepares or supervises preparation of clinic rooms and equipment for use as well as supports supplies ordering by maintaining inventories and participate as directed.
