Community Internal Medicine is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to become an integral member of our multidisciplinary primary care team. In this outpatient role, you will provide comprehensive care for established adult and geriatric patients with acute, chronic, preventive, and medically complex healthcare needs.


As an advanced practice provider, you will have the opportunity to build meaningful relationships with patients while working alongside experienced physicians and APP colleagues in a highly collaborative environment. This position utilizes a shared panel model, allowing you to partner closely with a physician or seasoned NP/PA to deliver coordinated, patient-centered care.


What You'll Do

  • Provide comprehensive assessments, evaluations, and ongoing management of adult and geriatric patients.
  • Diagnose and treat acute and chronic medical conditions.
  • Order, perform, and interpret diagnostic and laboratory testing.
  • Prescribe medications, treatments, and evidence-based therapeutic interventions.
  • Develop individualized treatment plans focused on quality outcomes and preventive care.
  • Educate and counsel patients and families regarding health conditions, treatment options, wellness, and disease prevention.
  • Collaborate with physicians, nurses, care coordinators, and other healthcare professionals to ensure seamless, coordinated care.

Qualifications

New Grads will not be considered at this time, 2+ years of APP experience required


Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current MN RN and APRN license and certification as a FNP, ANP or ACNP.  


Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the MN State Board of Medical Practice.  


Additional Qualifications: 
Active BLS certification from American Heart Association or American Red Cross at start date and DEA registration required
