We are seeking a talented and compassionate RNAC Registered Nurse Assessment Coordinator to join our team at Chestnut Hill Lodge Health and Rehabilitation Center, a 181-bed facility located in Wyndmoor, Pennsylvania. As a Registered Nurse Assessment Coordinator, you will play a vital role in ensuring the highest quality of care for our residents.

About the RNAC Registered Nurse Assessment Coordinator Role

As a Registered Nurse Assessment Coordinator, you will be responsible for coordinating the assessment process for new admissions, as well as conducting comprehensive assessments and updates for residents. You will work closely with the interdisciplinary team to develop and implement individualized care plans, and provide education and guidance to residents, families, and caregivers.

RNAC Registered Nurse Assessment Coordinator Responsibilities

  • MDS Coordination: Oversee the timely and accurate completion of Minimum Data Set (MDS) assessments required by federal and state regulations.

  • Care Planning: Partner with interdisciplinary teams (doctors, therapists, and social workers) to build and update individualized care plans for residents.

  • Reimbursement Compliance: Ensure proper documentation to support Medicare, Medicaid, and PDPM (Patient-Driven Payment Model) reimbursement.

  • Quality Monitoring: Track quality measures and indicators to spot trends and support facility improvement goals.

  • Audits and Surveys: Help prepare documentation and support the facility during state or federal regulatory audits.

RNAC Registered Nurse Assessment Coordinator Requirements

• Prior experience as an RNAC or MDS Coordinator required
• Current RN license in the State of Pennsylvania
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
