We are seeking a compassionate and dependable Hospice Registered Nurse/ Licensed Vocational Nurse in San Angelo  and surrounding areas to provide skilled nursing care to patients and families in the home setting. The RN/LVN will assess patient needs, manage symptoms, administer medications and treatments, provide education and support, and collaborate with the hospice interdisciplinary team to promote comfort, dignity, and quality of life.

Responsibilities
*Assess and monitor hospice patients. *Provide skilled nursing care and symptom management.
*Administer medications and treatments as ordered
*Educate and support patients and caregivers.
*Communicate changes in condition to the care team and physician*Maintain accurate clinical documentation. *Collaborate with the hospice interdisciplinary team. *Promote patient comfort, dignity, and quality of life.

Qualifications
Current, unrestricted RN/LVN license in the state of practice.
Graduate of an accredited nursing program.
Minimum of 1 year of nursing experience required.
Hospice, home health, or palliative care experience preferred.
Current CPR certification
TB test within the last 12 months

We are seeking a per diem After-Hours Nurse to provide skilled nursing and hospice care to maximize the comfort and health of patients. Schedule: Every Saturday & Sunday Coverage Area: San Mateo and Santa Clara County Essential Duties & Responsibilities include (but are not limited to): Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family. Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues. Education/Experience : One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
