Job Description

Purpose & Scope:

The Case Manager is responsible for the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes. Additionally, the Case Manager ensures that identified and referred patients have a coordinated continuum of care in accordance with JCAHO and hospital guidelines and serves as liaison for the interdisciplinary team, the patient, and family to ensure a timely, smooth, and clinically appropriate care planning process. The Case Manager also maintains communications with the pay or at the time of admission and throughout the hospitalization to ensure necessary approvals for care and clarification of benefits. They facilitate retrospective reviews and approvals for care whenever necessary, and they implement and coordinate discharge plans utilizing available and appropriate community resources.

Education:

Graduation from an accredited school of social work, nursing, in a related field or equivalent is required.

Experience:

Five years of experience in the healthcare field is preferred.

Experience in case management is preferred.

Job Overview In accordance with Virginia School Health Guidelines, Student Health Services provides care to ill and injured students, administers medication, performs vision and hearing screenings, trains school personnel to safely care for students with medical concerns, and reviews physicals and immunizations for school entrance. Renders limited first aid; checks temperatures; contacts parents and school personnel as necessary regarding medical issues. Manages daily care of students with chronic medical concerns according to the individualized health care plans. Documents student clinic visits and care provided daily on the computer; initiates and completes injury/incident reports as required. Maintains an organized clinic with necessary items to provide routine first aid and respond to emergencies. May perform state-mandated screenings and prepare reports. Provides direct nursing service for routine and emergency situations and prepares and maintains medical records. Applies considerable judgment and interpretation in unusual or nonstandard situations. Confers with and advises school administrators regarding student health issues, including student medical concerns, parent medical concerns, health emergency procedures, and special education needs. Collaborates with parents, medical providers, and LCPS staff to manage health issues safely and appropriately at school. Performs related work as required.
