Are you the nurse everyone calls when a wound isn't healing? At Port Washington Post Acute, you'll own our skin and wound care program. You'll be the go-to expert who prevents pressure injuries, leads treatment plans, and shows how much healing makes a difference for our residents. What You'll Do
Lead the facility's skin and wound care program, including assessment, treatment, and prevention of pressure injuries and other skin conditions
Perform wound assessments, staging, and measurements, and track healing progress
Partner with physicians, the Medical Director, and the Director of Nursing on treatment plans and referrals
Collaborate with the interdisciplinary team on care plans and skin-related assessments
Train nursing staff and CNAs on skin care, prevention techniques, and wound care best practices
Educate residents and families on skin care, especially before discharge home
Keep wound care supplies and treatment products stocked
Maintain accurate, timely documentation and support state survey readiness
Advocate for residents' dignity, rights, and quality of care

What You'll Need
Current, active Washington RN or LPN license
Experience in long-term care, skilled nursing, or post-acute settings
Wound care experience, including assessment, staging, and treatment
Wound care certification (WCC, CWCN, or similar) is preferred but not required
Strong communication skills with residents, families, and the care team
Ability to work independently and make sound clinical decisions
