About the Role of LPN for SNF:

As an LPN with Deer Meadows, you will play a vital role in our Skilled Nursing Facility (SNF) team, providing high-quality patient care to our residents.

LPN for SNF Responsibilities:

Providing direct patient care in accordance with our facility's policies and procedures
Conducting vital signs, wound care, and other basic nursing procedures
Maintaining accurate and timely records of patient care and treatments
Collaborating with the interdisciplinary care team to develop and implement individualized care plans
Providing emotional support and comfort to our residents and their families
