UVA Health

Registered Nurse Care Coordinator (RNCC) - Digestive Health Clinic

$88,212.80 - $115,793.60 / year

Bring your clinical judgment, curiosity, and patient advocacy to a role where expert nursing coordination can shape the entire care experience for people living with complex gastrointestinal conditions.

As a Registered Nurse Care Coordinator within UVA Health’s Digestive Health Clinic, you will serve as the clinical connection point between patients, providers, and essential services. This opportunity combines meaningful patient relationships, independent nursing practice, specialty development, and a predictable hybrid work structure.

The Role

You will help patients with Crohn’s disease, ulcerative colitis, celiac disease, chronic abdominal pain, and other complex gastrointestinal conditions to navigate diagnostic evaluation, treatment, and long-term disease management. Rather than performing isolated tasks, you will assess evolving needs, investigate clinical concerns, and help translate provider care plans into coordinated action.

Your average day may include:

  • Coordinating diagnostic testing, procedures, laboratory monitoring, and follow-up
  • Managing medication questions, refill requests, and prior authorization support
  • Responding to patient concerns by phone and MyChart using sound nursing judgment and critical thinking
  • Educating patients about care plans, testing, medications, and next steps
  • Coordinating care across GI providers, specialty services, and multidisciplinary teams
  • Identifying clinical issues, gathering relevant information, and independently advancing appropriate solutions
  • Building trusted relationships with patients who require ongoing monitoring and support
  • Partnering with physicians, nurses, and clinic staff in a collaborative, team-oriented practice

The position follows a Monday through Friday, standard business hours schedule with no nights, weekends, or major holidays. A consistent hybrid model combines onsite clinical collaboration with remote work aligned to provider schedules. Schedules are generally established in advance, with flexibility expected to support clinic coverage when team needs change.

GI experience is valuable, but nurses from medical-surgical, ICU, acute care, ambulatory specialty, infusion, chronic disease management, and existing care coordination backgrounds may also bring highly transferable skills. Strong recent clinical experience, organization, communication, and independent problem-solving skills are essential.

Why UVA Health

UVA Health offers the depth of academic medicine with the connection and shared purpose of a highly collaborative specialty practice. The Digestive Health Clinic is a growing multidisciplinary environment that brings together expertise in inflammatory bowel disease, hepatology, colorectal surgery, and general gastroenterology to support patients with complex and often chronic conditions.

In this role, you will have the opportunity to:

  • Develop specialized expertise in gastroenterology
  • Exercise meaningful autonomy while remaining connected to a supportive clinical team
  • Take ownership of complex patient needs and help move care plans forward
  • Work alongside physicians and staff who collaborate as partners and actively support one another
  • Grow within an expanding specialty service where professional development is encouraged
  • Build lasting patient relationships and make a direct impact through education, coordination, and follow-up
  • Contribute to a professional nursing environment grounded in interdisciplinary collaboration, evidence-based practice, and continuous learning
  • Pursue career development through nursing education, mentorship, recognition, shared governance, and specialty certification resources

This is an opportunity for a clinically curious nurse who wants to move beyond episodic care and become a trusted resource throughout a patient’s healthcare journey.

Relocation assistance may be available for qualified candidates.
