Job Summary:

As an LPN at Bridgewater Center for Rehabilitation and Nursing, you will be responsible for providing high-quality nursing care to our residents under the supervision of our experienced medical and nursing staff. Your duties will include:

Administering medications and treatments as ordered by physicians
Assisting residents with daily living activities, such as bathing, dressing, and grooming
Monitoring and reporting residents' conditions to the nursing team
Collaborating with the interdisciplinary care team to develop and implement individualized care plans
Providing emotional support and companionship to our residents

Communicates with direct care staff with all resident care concerns at the beginning and throughout the shift. Answers families’ and visitor’s questions as necessary. Is responsible for emergency equipment and general nursing supplies; restocking medication/treatment carts and any other supplies used. Participates in care plan meetings when asked. Makes rounds with physicians. Reports all accidents and incidents to RN supervisor and documents on resident chart and on incident reports; informs doctor and family.
