Applicants must provide their complete availability so they can be considered for appropriate assignments. Once a caregiver accepts a permanent, temporary or PRN assignment, dependable attendance and completion of the entire scheduled shift are required. Client Care Levels Helping Hearts supports clients with different levels of need. Assignments are matched according to the caregiver’s experience, skills, availability, location and ability to safely complete the approved care plan. Responsibilities may include: Tidying the client’s immediate living space Dusting Sweeping Vacuuming Mopping approved areas Washing dishes Cleaning frequently touched surfaces Laundry and linen changes Cleaning the client’s bathroom Organizing care supplies Removing trash Keeping walkways and emergency-access areas clear Reporting unsafe conditions or needed repairs Housekeeping responsibilities are limited to the client’s approved service area and assigned care-plan tasks. Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Communicate professionally with clients, office staff and approved facility personnel. Complete accurate electronic documentation during every shift. Have reliable access to a smartphone. Maintain client confidentiality. Maintain professional boundaries. Remain attentive and awake throughout every assigned shift. Follow attendance and call-off procedures. Transportation Requirements Applicants must have: A valid driver’s license Current automobile insurance Reliable personal transportation An acceptable driving record The ability to travel to assigned client locations The ability to transport clients when required and authorized Public transportation, rideshare services or transportation provided by another person may not satisfy the requirements of assignments requiring immediate or independent travel. Caregivers must immediately report any change affecting their driver’s license, insurance, vehicle availability or ability to transport clients safely. Attendance and Communication Expectations Dependability is essential in home care. Caregivers must: Arrive on time for every accepted assignment. Confirm assignments according to company procedures. Maintain reliable telephone communication. Respond promptly to scheduling messages. Notify the office immediately if an emergency affects attendance. Follow all call-off procedures. Clock in and out from the approved client location. Remain with the client until the scheduled end of the shift. Remain until properly relieved when continuous care is required. Never leave a client unattended when supervision is required. Never leave early without office authorization. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Documentation Requirements Caregivers must document services accurately and in real time. Background and Screening Requirements Applicants must: Complete a comprehensive background check covering up to 15 years, where legally permitted. Complete required abuse, neglect and sex-offender registry checks. Complete a pre-employment drug screening when applicable. Participate in random drug screening when required by company policy. Provide complete and accurate application information. Report criminal charges or legal changes that may affect their ability to provide safe care. Have no disqualifying history that would prevent safe employment, subject to applicable laws and individual review. Employment offers are contingent upon satisfactory completion of required screenings, employment verification, reference checks and onboarding requirements. AWAKE OVERNIGHT POSITION – ABSOLUTELY NO SLEEPING For caregivers working 7:00 PM–7:00 AM, this is an AWAKE overnight assignment. Sleeping while on duty is strictly prohibited. Night-shift caregivers must remain awake, alert, responsive, attentive, and available to the client throughout the entire 12-hour shift. Sleeping, napping, dozing, intentionally lying down to rest, or otherwise becoming unavailable to the client is not permitted. Caregivers may not sleep on the client's couch, recliner, bed, chairs, floor, or other household furniture. Caregivers may not use the client's blankets, pillows, bedding, or other personal belongings for sleeping or personal comfort. The fact that the client may be sleeping does not mean the caregiver's responsibilities stop. Overnight responsibilities may include: Regular client checks according to the care plan. Experienced Caregiver Needed for Franklin-Area Client Location: Franklin, Tennessee Schedule: Every Monday, Wednesday, and Friday Hours: 12:00 PM–5:30 PM Weekly Hours: Approximately 16.5 hours Position Type: Consistent, part-time home-care assignment Helping Hearts is seeking a reliable, compassionate, experienced, and highly attentive caregiver to support a client in the Franklin area. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Responsibilities may include: Bathing or personal-hygiene assistance Dressing and undressing Grooming and oral-care support Toileting and incontinence care Mobility assistance Positioning and repositioning Safe transfer assistance Wheelchair and mobility-device assistance Meal preparation and hydration reminders General comfort and personal-care support Maintaining the client’s privacy and dignity during all care tasks Care must be provided patiently and professionally without rushing the client. The caregiver must follow the established care plan and use only approved transfer techniques and equipment for which the caregiver has received proper training. Safe Transfer Ability Is Required This client requires hands-on assistance with transfers. Applicants must have the physical ability and willingness to safely assist the client with transfers, including: Wheelchair to commode and commode to wheelchair Wheelchair to bed and bed to wheelchair Wheelchair to chair or other approved seating Sit-to-stand and stand-to-sit transfers Toileting-related mobility and clothing-management assistance Repositioning for safety, hygiene, and comfort Transfers using approved equipment when required by the care plan Previous experience using a Hoyer lift, sit-to-stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. If the client cannot be moved safely according to the care plan, the caregiver must protect the client from further injury and contact the Helping Hearts office immediately for direction. Active Engagement and Companionship The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment.
