Description Kindred Hospital Chicago - Northlake is a 94-bed long-term acute care hospital offering the same in depth care you would receive in a traditional hospital, but for an extended recovery period. We partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. We are located in the city community offering a nice balance between residential and commercial area, close to Belmont Gardens. Job Summary Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel. Essential Functions Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care. Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift. Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner. Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assists in patient admission, transfer, and discharge procedures. Assist with handling and care of patient belongings and other personal property. Answers patient call lights, telephone, and paging system and responds as appropriate. Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold. Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening milk container, cutting food for patient. Knowledge/Skills/Abilities/Expectations Must read, write and speak fluent English. Must have good and regular attendance. Excellent oral and written communication and interpersonal skills Basic computer knowledge. Able to organize tasks, develop action plans, set priorities and function under stressful situations. Ability to maintain a good working relationship both within the department and with other departments. Approximate percent of time required to travel: Performs other related duties as assigned. Pay Range: $18.00-$26.75/hr. Be considerate, pleasant, and professional in the performance of his/her duties. Must have compassion, tolerance, and understanding for the residents of the facility. Knowledge of JCAHO, OBRA, IDPH, and HFS regulations as related to duties. MAIN DUTIES: Support the facility's philosophy of care and strive to achieve its goals and objectives. Be sensitive to residents' families and respond in an appropriate professional way as the situation requires. Carry out assignments for resident care including (but not limited to): a) bathing b) dressing c) grooming d) shaving e) feeding f) restorative nursing procedures g) retraining Keep residents' bed, dresser, bathroom, and general living area clean and tidy. Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures. Perform other related duties as directed by the DON, ADON, charge nurse, or the Administrator. Overview • Department: Acute Rehab • Schedule: Nights, 7p-7a, part-time, weekend and holiday rotation • Facility: Prime Healthcare Saint Joseph Medical Center • Location: Joliet, IL 60435 Responsibilities The Patient Care Assistant assists patients with activities of daily living, provides basic nursing care to all patients and assists in maintenance of a safe environment under the direction and supervision of the Registered Nurse in charge of the team and/or unit. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current Phlebotomy certification upon hire, preferred.2. Current EKG certification upon hire, preferred.3. Current BLS (AHA) within 30 days of hire and maintain current. 4. Possesses knowledge of Medical Terminology.5.
